Required Documents
To open a case and assess the conditions, the following documents (as applicable) are collected from the patient:
- Identification documents
- Medical records and case file
- Rental agreement (if renting)
- Pay slip or income documents
- Confirmation from the relevant city charity (if available)
- Confirmation from the village council or headman (if residing in a rural area)
- Imam Khomeini Relief Foundation card (if covered)
- State Welfare Organization card (if covered)
- Death certificate of the head of household (if deceased)
- Home visit report (if needed)
- Confirmation from the attending physician
- Guardianship document (if a guardian exists)
- Other miscellaneous and supplementary related documents
Key Criteria in Scoring and Support Percentage
In the scoring and interview process, several key items are considered that can directly affect the percentage and type of support:
- If the patient has special insurance booklets such as those from the Ministry of Energy, banks, IRIB, and similar entities, they are typically not eligible for social work support because all expenses are paid by that organization (i.e., the patient effectively has supplementary/organizational coverage).
- If the patient has government employee insurance, Social Security pensioner insurance, or armed forces insurance, they are usually not entitled to social work support due to having supplementary insurance or insurer support.
- If the patient has supplementary insurance but the insurer’s commitment ceiling has been exhausted, they may receive temporary and limited support until the ceiling is reset (e.g., until November), while being guided on how to renew the ceiling.
- If the patient has any of the following insurance types, they are recognized as a client, a case is opened, and the normal support process proceeds:
- Rural insurance
- State Welfare Organization
- Imam Khomeini Relief Foundation
- Iranians insurance
- Worker Social Security
- Voluntary Social Security
- Drivers’ Social Security (without supplementary insurance)
- Carpet weavers’ Social Security
- The following social and economic indicators are important factors in determining the percentage and type of support:
- Being a tenant
- Lacking a head of household
- Being a client of the Imam Khomeini Relief Foundation or the State Welfare Organization
- Having school-age children under 18 (with no possibility of employment)
- Having a disabled child in the family
- Being a widow
- Income and circumstances of the head of household
- Length of treatment period and periodic treatment costs
- Financial, livelihood and income situation of the family
- Family asset status (such as livestock, agricultural land, owner-occupied home, salaried status, etc.)
- In addition to support from the Association’s Social Work Unit, the patient is definitely referred to the charity in their city of residence for supplementary assistance.
- If eligible, the patient is introduced to the State Welfare Organization and the Imam Khomeini Relief Foundation for coverage, and the necessary coordination with these organizations is carried out.
- The patient is also referred to the Food and Drug Deputy Office for government assistance, so that after benefiting from government support, they can enter the social work support cycle at minimal cost and also use public financial aid.
- If a specific organization, institution, hospital or treatment center is willing to help, the patient is referred to those entities for support, including:
- Referral to partner charities (e.g., Ashura Aid Charity)
- Booking appointments at public centers
- Obtaining discounts from treatment centers
- Writing letters to confirm the patient’s condition and secure discounts or support
- Coordinating with Tehran charities for specialized services in that city
- A home visit is one of the most fundamental actions to verify the accuracy of the applicant’s statements and is carried out by the respective social worker. This is especially conducted with greater sensitivity for patients who receive more than 50% support, or in cases where contradictory information was provided during the interview.
